Insurance
We accept Medicare Part B. Unfortunately, that precludes individuals with managed Medicare or Advantage Plans from other insurance companies. Prior to starting services, we will verify your coverage.
Out of Network
We are happy to provide you with a bill for you to submit to your insurance carrier for reimbursement. The bill includes an itemized list of services provided, as well as diagnostic and procedural codes. There is no guarantee that your insurance provider will reimburse you for the full or even partial amount of the bill. We ask that prior to starting services, you contact your insurance provider to obtain information on your Out of Network benefits, restrictions, and reimbursement for the following codes:
96116 (estimated 1 unit)
96132 (estimated 1 unit)
96133 (estimated 2 units)
96136 (estimated 1 unit)
96137 (estimated 7 units)
At the time of evaluation, you should plan to pay for services in full. Following your evaluation, you can then submit your bill to your insurance provider to support your claim.
Self-Pay
Paying for services out-of-pocket has benefits. Insurance companies often request personal health information and sometimes place restrictions on the services you are eligible to receive. The self-pay option allows you greater flexibility in service and privacy.
